- Patient's portion is due at the time services are rendered.
- Membership cannot be used in conjuction with other dental insurance or dental discount plans.
- Membership cards will be issued, however, full membership plan details will be attached electronically to the patient's file.
- Membership plan cannot be used for injuries that fall under workman's comp claim.
- Your membership is not transferrable to friends and/or family members.
